I really wish the documentation was better for those of us still fumbling our way around Joomla. I have some skills but I am certainly better at running my business than trying to get my website setup the way I want it, that is for sure.

This page has the elements that I am trying to duplicate:

And I believe that if we could just have a screen shot of the manager and options pages that sets up that contact form, we could figure out how to set ours up.

Can you help us out here please? I have looked for documentation but there appears to be very little available.

You can find something even better than a screenshot. The back-end of our demo section is open for public. The username is demo and password is demo
You can just login at http://demo.mavxr.com/administrator and find out whatever you need on how the fields are configured.
