It depends on which items you are trying to add.
If you are adding Joomla articles, K2 items, Virtuemart items and a few more, thumbnails are supported by default. If you are using one of those types and thumbnails do not appear, it means that you are not adding images in the default way. MyShortlist does not extract the first image from the article. It uses the images set in the "Images" tab.
If you are using other types of items, then you may need to create your own template override for this specific item type, to make it look exactly as you want it. Each website is different and each developer uses their own way of styling. You should customize MyShortlist for your needs.
