Just a suggestion from a problem I encountered recently.
I added a new quiz, all radio buttons, and when I tested it, no correct answers were recorded.
Problem was I had a space in the line where the [option1/] was in the Correct Answer for the question.
Not too hard to figure out, but not obvious. You might add trimming that to your list.
